DESPITE medication and guidelines to control diabetes a majority of patients have poor control over blood pressure and cholesterol Now researchers at the All India Institute of Medical Sciences (AIIMS) the Public Health Foundation of India (PHFI) and the Rollins School of Public Health Emory University Atlanta have found that a low-cost care intervention treatment model can help patients double their likelihood of controlling diabetes For the study published in the Annals of Internal Medicine researchers analysed results from a trial conducted at ten clinical centres in India and Pakistan They compared the effect of a multi-component diabetes-focused quality improvement (QI) strategy against the usual care alternatives for heart patients with poorly controlled diabetes Nikhil Tandon Professor and Head Department of Endocrinology AIIMS the senior author for the publication said the team compared the effects of multicomponent diabetes QI strategy (combining a non-physician care coordinator and decision-support electronic health record software) with the usual care in South Asia where resources are limited and diabetes is prevalent Approximately 1150 patients with diabetes and poor cardio-metabolic profiles were randomly assigned to either the multicomponent QI strategy or the usual care for two and a half years Results suggested that patients in the QI strategy group were twice as likely to achieve combined diabetes care goals and larger reductions for each risk factor compared to the usual care “This was the first trial of comprehensive diabetes management in a low/middle-income country setting” explains Tandon “This intervention does not require new or expensive drugs but instead it enhances a patient’s likelihood of managing their disease on their own by providing individualised support and enhancing the physician’s likelihood of being responsive” Share This Article Related Article Watch Video: What’s making news Tandon explained that people with diabetes need to control blood sugar cholesterol and blood pressure There are medications for all three and standard guidelines to manage them; despite this a majority patients with diabetes have poor control with blood pressure and cholesterol It is this suboptimal standard of care which the intervention addresses — using the usual medication and a real life setting wherein the patients paid for their visits consultations medicine and tests So this really targets improvement in care delivery at the clinic level Though this study was done at tertiary care level we have replicated it at community health centres (secondary care level) also Dr Tandon further added “By better controlling their blood sugar blood pressure and cholesterol levels our study offers a hope of reducing onset of diabetes complications like heart disease eye disease kidney failure and amputations which are very common in people with diabetes in South Asia” Dr D Prabhakaran Vice President Research and Policy PHFI told The Indian Express “Since these findings are relevant to India and many other countries further research will uncover whether this approach reduces diabetes complications such heart attacks strokes eye disease kidney failure and amputations in the long-term and to assess patients’ and providers’ views so that the intervention can be delivered more widely” he added Of the 415 million people with diabetes worldwide 75 per cent live in low/middle-income countries India alone is home to the second highest number of people with diabetes (nearly 70 million) worldwide Tandon quoted a DiabCare India study (2011) to state that a proportion of patients with diabetes in India are currently not able to maintain desired blood sugar profiles The 2011 study had showed that mean HbA1c (a measure of average control over 3 months) was 89 per cent in the country with less than 20 per cent of people meeting the glucose target of 7 per cent A similar study conducted a decade earlier had shown that about 75 per cent of people attending diabetes clinics had an HbA1c at 7 per cent a clear indication that glucose control is poor and that there have been no major improvement over the last 10-15 years He further reiterated that it was a quality improvement strategy in which the intervention is a composite of a non-physician care coordinator (akin to a community health worker – but stationed in a clinic and a decision support software which is embedded on a platform provided by an electronic health record For all the latest Lifestyle News download Indian Express App human sperm cells and some mimic the effect of the female hormone progesterone Sunscreen impairs sperm function by seeping through the skin and into the rest of the body the researchers explained Read More UV-filtering chemicals were found in almost all urine samples and some blood samples “These results are of concern and might explain in part why unexplained infertility is so prevalent” said one of the researchers Niels Skakkebaek professor at the University of Copenhagen in Denmark To examine how UV-filtering chemicals affect healthy sperm the team tested 29 of the 31 UV filters allowed in sunscreens on healthy human sperm cells in a buffer solution that resembled the conditions in female fallopian tubes The team concentrated on calcium signalling – particularly the sperm-specific calcium ion channel called CatSper which binds to the female hormone progesterone to control sperm cell fertilisation functions like sperm motility The results revealed that 45 per cent of the 29 UV filters tested in the study interfered with the normal functioning of the sperm cell Also 13 UV filters triggered a surge in the movement of calcium ions within sperm cells during binding with passing of my Guruji Sitara Devi I had the honour of becoming her first American disciple and living with her in Mumbai while on a Professional Development Grant from The American Institute of Indian Studies in 1983-84 For me in addition to being my Kathak Guru she brought me into a deep experience and understanding of the beauty and power of the spiritual tradition of Hinduism and the Vedas and particularly the beauty power and grace of the Divine Feminine Her Shakti still reseonates through my life and art The following is an excerpt from my book The Path of the Priestess: A Guidebook for Awakening the Divine Feminine which brings the depth of her spiritual teachings to light One evening as we sat together in her living room I looked up to see a very powerful image of a beautiful Goddess with many arms who was dressed in red and seated gallantly astride a lion “Please Guruji” I asked “tell me about the Goddess She seems to be so important in your life In the Jewish tradition of my birth as well as the Christian religion that dominates the West we are told that there is only one God and he is definitely male” Sitara closed her eyes for a moment and quieted her breath as if going into a deep meditative state The atmosphere of the whole room seemed to change The power and energy that flowed through Sitara was so strong that chills ran up my spine She began to speak her voice filled with passion “There is a sacred current of light that flows throughout all of creation” she explained “It is the animating and nourishing force of life itself the supreme all-illuminating power in every thing and every being We call this light and energy Shakti or the Great Goddess As the creative power of Shiva Shakti is the vital force in all beings; there is nothing in the world that is devoid of her presence Shakti is life and gives life to all She is bliss and gives bliss to all Even though she is essentially one in order for us to know her she manifests herself in many forms She is the voice light and energy of what you in the West call the Godhead Many of our ancient texts known as the Vedas Upanishads Puranas and Tantras sing her praises Watch carefully and I will reveal her to you” After Sitara spoke these words she went through an almost unbelievable series of physical transformations before my astonished eyes As she named each goddess and brought her to life it seemed as if she could magically alter the shape and size of her body Not only could she shape-shift in this remarkable manner but also it appeared that she had the ability to transform her very flesh so that with her depiction of each successive goddess the small wrinkles around her eyes magically disappeared and she actually became a younger version of herself First Sitara manifested as the fierce and powerful goddess Durga whose image adorned the wall Riding astride a lion Durga held in her many hands the weapons with which she destroyed the terrible demon Mahisha The energy that exuded from Sitara was so compelling her movements and gestures so powerful that I could actually see these multiple hands as if they were real In the blink of an eye Sitara then transformed herself into the warm sensuous Lakshmi goddess of fertility and abundance Light seemed to radiate outward from her body as her breasts appeared to swell and brim over with life-enhancing nectar In one hand she held a lotus flower while sparkling gold coins poured out of the other hand Sitara looked at me and laughed with delight as she became the youthful and serene Saraswati goddess of music singing softly to herself while playing her sacred instrument the Vina Finally Sitara transformed into Kali Ma the great wrathful goddess who with her shining sword destroys egotism and ignorance The skin of Sitara’s face tightened until her face began to resemble a skull Her ancient all-knowing eyes were wild with fire and her red tongue hung out of her mouth A third eye suddenly appeared between her two eyes as she looked at me with great ferocity I gazed into its depths and for a moment it was as if I was looking into the fiery core of creation itself I was overcome with awe and amazement This was a spectacle and energetic transmission of knowledge unlike anything that I had ever witnessed Sharron Rose is a true renaissance woman She is a filmmaker author performer teacher and much more From said Thus far the GRP’s imagery has been restricted to posters at railway stations with collages of dead men and warnings to not cross tracks on foot he said Policemen and women are central characters in most images but Talan said he ensured that they were also seen in their primary function staying in the background to secure coaches reserved for women mingling with commuters at railway stations waiting to board trains and in most cases – simply being present Talan added that he tried to make the images in the calendar as relatable as possible and not larger than life “The most popular pictures of CST show crowds moving around I reversed that to show a train moving but the police still present” he said Some of Talan’s pictures include familiar locations such as Churchgate Mumbai Central Andheri Sandhurst Road Karjat and Turbhe stations while some of the most memorable pictures on the calendar were shot on Bhayander Creek bridge Thane’s Parsik Hill tunnel and Kasara Ghat The photographer said he attempted to convey the scope of the GRP’s jurisdiction and the enormity of the assets they protect “In these troubled times when cases of rail sabotage are being reported the police are required to be far more vigilant” he said Accordingly the calendar has pictures of policemen posted inside Parsik tunnel patrolling the Kasara ghat bridge and standing atop the Bhayander
